Life Insurance

County Provided

Douglas County provides benefits eligible employees with a $15,000 Basic & AD&D group term life insurance plan through Voya Financial.  A new employee can elect optional life coverage up to six times their salary with a limit of $300,000 without providing evidence of insurability.  To learn more about the Life Insurance benefits, just click the link below.

Whole Life Insurance

Mass Mutual also offers a whole life policy to County employees.  For premiums or more information about the policy, contact Dave Beck at 402-658-9139.

 

Optional Term Life Insurance

A new employee has guaranteed issue optional life coverage up to six times their salary or $300,000 whichever is less.

During Open Enrollment, employees may complete an evidence of insurability form and request an increase in optional life insurance. The limit is 6 times annual salary or $500,000, whichever is less.  

The optional life insurance premiums are deducted on the first check of each month. 

Age Reductions:

​Benefit reductions happen every five (5) years beginning on first of the month following age 65. Details are in the rate tables below. 

Retirement Reductions

Retiree coverage terminates at age 65​.

Spouse Life Insurance Coverage

Spouse life insurance is guaranteed issue in amounts of $10,000 or $20,000 NOT to exceed 50% of the employee’s optional life amount for new employees. 

During  Open Enrollment, an evidence of insurability form may be completed to request an increase in spouse life insurance coverage. The limit is 50% of the employee’s optional life coverage with a maximum of $250,000.

Child Life Insurance

Child life insurance is guaranteed issue in increments of $2,000 up to a maximum of $10,000 for new employees. Children are eligible from birth up to the attainment of age 26.

During each Open Enrollment, an evidence of insurability form may be completed to request an increase in child life insurance coverage.
